Neurosonogram in Critically Ill Neonates in Neonatal Intensive Care Unit

چکیده
Aim: Neonates born prematurely and sick full-term neonates are at risk of brain injury. Although advances in neonatal intensive care have greatly improved the survival and outcome of these vulnerable patients, brain injury remains of major concern. Early diagnosis is important for prognostication, optimal treatment, and predicting the neurological outcome. Objective: This study was done to describe the pattern of cranial ultrasound abnormalities in preterm and term critically ill neonates in neonatal intensive care unit (NICU). Materials and Methods: This prospective observational clinical study was done in Vydehi Institute of Medical Sciences Hospital and Research Centre, Bengaluru between January 2014 and July 2015. After obtaining informed consent 100 critically ill neonates admitted to our NICU were included in this study. History and clinical examination followed by appropriate investigations were done. These critically ill neonates were subjected to neurosonography on selected days as per as protocol and different patterns of morphology abnormalities were noted. Clinical correlation with neurosonogram findings was observed and if found abnormal follow-up neurosonogram were done. Results: The incidence of neurosonographic abnormalities in high-risk neonates is 31% in the present study. Of these 41% of these had evidence of intracranial bleed, 25% had cerebral edema, 6% periventricular leukomalacia, 16% hyperechogenic thalami, and one had ventriculomegaly. Of the 31% of neonates with abnormal findings on neurosonogram, 22% had hypoxicischemic encephalopathy as per Apgar scoring, 25% had features of sepsis. One neonates with intraventricular bleed on regular follow-up of neurosonogram developed ventriculomegaly. Conclusion: This study signifies the importance of neurosonogram in critically ill neonates as diagnostic tool and as screening modality in NICU. It also emphasizes its use as a screening modality for preterm and birth asphyxia neonates influencing their neurodevelopmental outcome. Neurosonogram is critical as an investigatory modality in NICU for early, safe and easy diagnostic tool for predicting the neurological damage for management in NICU and predicting outcome.

Evaluation of Nutritional Status in a Teaching Hospital Neonatal Intensive Care Unit
Background:Extrauterine growth restriction remains a common and serious problem in newborns especially who are small, immature, and critically ill. Very low birth weight infants (VLBW) had 97% and 40% growth failure at 36 weeks and 18-22 months post-conceptual age respectively. The postnatal development of premature infants is critically dependent on an adequate nutritional intake that mimics a...
متن کاملA comparison of CRIB, CRIB II, SNAP, SNAPII and SNAP-PE scores for prediction of mortality in critically ill neonates
Abstract Background: Clinical Risk Index of Babies (CRIB), Score for Neonatal Acute Physiology (SNAP), an update of the Clinical Risk Index for Babies score (CRIB II) and Score for Neonatal Acute Physiology - Perinatal Extension (SNAP-PE) are scoring devices developed in neonatal intensive care units. This study reviewed these scoring systems in critically ill neonates to determine how well th...
متن کاملProphylactic Ranitidine in Prevention of GI Bleeding in Neonatal Intensive Care Units
Background:Upper gastrointestinal (GI) bleeding remains a problem in critically ill-newborns. The use of H2 blockers, by maintaining gastric pH ³ 4, reduces the risk of stress-ulceration and gastric hemorrhage. This study therefore, has evaluated the effects of short-term prophylactic ranitidine in controlling gastric pH and prevention of GI bleeding in neonates. Methods: This randomized contr...

OBJECTIVE To identify the optimal time for introducing enteral nutrition to critically ill neonates. METHODS This prospective cohort study included all eligible critically ill neonates who were admitted to a multidisciplinary tertiary neonatal intensive care unit (NICU) between 1st June and 30th November 2013. Nutrient intake and clinical outcomes during NICU stay were recorded. The effect of...
متن کاملAssociation between admission hypomagnesemia mortality or mortality of critically ill patients in intensive care unit
Background: Up to now there is no study evaluating correlation between serum magnesium and morbidity or mortality in patients admitted in intensive care unit. The aim of this study is to determine the prevalence of hypomagnesemia in critically ill patients and to evaluate its association with organ dysfunction, hospitalization period and mortality. Methods: We conducted a retrospective trial ...
